Description | Human Services Clinicians make home visits; develop plans of care to reduce risk for residents age 60 and older and those who are homebound with multiple disabilities. This program is also focused on preventing and treating abuse, neglect or exploitation of older adults. When remaining in the home isnt feasible for some older adults, Human Services Clinicians assist in researching and securing new living arrangements. There is no fee for Adult Social Services Case Management. |

Eligibility Details | Age 60 or older and unable to effectively advocate for him or her self. If under 60, person has a disability and is unable to leave the home and manage activities of daily living nor advocate for self effectively. Anyone requesting a screening for eligibility to the CC+ waiver (formerly the Elderly and Disabled with Consumer Direction Medicaid Waiver). |
