Please start the process by contacting our Assessment and Coordination Nurse at x1715 to discuss the potential nursing case management needs of your client. She will give you details about the next step in the process. She will e-mail a copy of the referral form, as needed.
Please know potential clients need to be aware of and consenting to nursing case management services prior to making referral.
The client should be made aware that there is a monthly fee for services based on income minus medical and dependent care expenses.
If clients are appropriate for services, an initial joint home visit between the client, referring case manager, and ADSD nursing staff will be needed for maximum care coordination.
The referring case manager and the nurse case manager will periodically check in with each other to ensure the care coordination needs of the client are being met. Please know the nurse case manager will not have routine access to the CSB electronic record.

A monthly fee, based on a sliding scale is assessed and billed monthly. Note: This is the same monthly fee scale for case management used by the CSB. It takes into consideration gross income, minus medical expenses.
